Google docs allows you to create & embed questionnaires into your wiki, blog or other website. The questionnaire will display as a form and may have formatting added to change fonts or background images. A variety of scales or answer types are also available which include; drop down boxes, check boxes, text short or paragraph text, multiple choice answers, radio buttons and scales from 1 to 10. Answers may be made compulsory with consolidated responses recorded within a spreadsheet and made available immediately for viewing if desired.
The only restriction I have found thus far, and this may be due to my limited experience in working with Google Docs, is that the width of the document cannot be adjusted to comply with narrow column formatting provided within Blogger. Subsequently, I have included a link so that my example form may be viewed; however, please explore the "embed" option available from the toolbar titled "more actions".
